Term & Conditions For Entity registration request 
The end of service must be in accordance with the reasons for termination specified in Law No. 5 of 2018 concerning social security in the Emirate of Sharjah.
Service Requirements

Completion of the End of Service Form.

End of service procedure.

Employment Commencement Form copy.

Subscription statement for the last three years or the entire subscription period if less, including any changes.

ID card copy with family members (if married).

Pre-employment and post-service registration extract.

Naturalization confirmation letter from the Ministry of Interior if applicable.

Bank account certificate (IBAN).

Proof of service consolidation or purchase, if applicable.
